Mike O’Hearn insists that his sculpted body was built the old-fashioned way with training and nutrition, devoid of performance-enhancing drugs. In a recent YouTube video published on November 7, 2023, O’Hearn joined influencer Kenny KO to discuss whether or not testosterone replacement therapy makes someone unnatural.
“The reunion that everyone was waiting for, you told me you’d come back [last year],” Kenny KO shared.
“I didn’t want to get busted guys,” said O’Hearn.
Having tested his strength in powerlifting, strongman, and bodybuilding, O’Hearn is no stranger to the dedication and willpower necessary for competing at the highest level. In addition to those ventures, he’s been featured as a model, published in countless magazines, and also made a splash in Hollywood with TV and movie appearances.

What makes O’Hearn so fascinating is his longevity and shredded body at 53 years old. For decades, O’Hearn has boasted a detailed and near-contest-ready physique, defying expectations and time. Even though there have been widespread accusations about his ‘lifetime natural status,’ O’Hearn has never failed a drug test.
Mike O’Hearn Defends ‘Lifetime Natural’ Status w/ Kenny KO: ‘Never Came Up Negative’
Kenny KO had a few questions for O’Hearn who claims to be a lifetime natural but was seen promoting a hormone-optimizing clinic called Titan Medical. Again, KO asked O’Hearn about his use of TRT:
“The one big question I have for you this expo is, do you still claim lifetime natural?” asks Kenny KO. “My big question if you are claiming natural, I assume this company laying around is a hormone optimizing clinic, so do you take TRT?” asks KO.
“Yes, it is. Yes. Natural is natural, right? And any kind of TRT or HRT would be unnatural right? Because you’re adding something to it, is that what people are doing now, ‘Hey I’m natural, but I’m on TRT,’?” O’Hearn asked.
According to O’Hearn and KO, someone taking TRT is unnatural despite some influencers suggesting otherwise.
“If you’re on stuff, you’re on stuff, if you’re not, you’re not,” added O’Hearn.
“I think if you’re taking any foreign hormones into the body even if it’s to replenish what you think you need or a doctor says you need, you’re still not natural because it’s an exogenous source that’s coming in,” said KO.
“It’s helping you, you’re optimizing your body, trying to be the very best you can, and you’re on TRT right now? And that’s good, and you feel better right? Three-ish years ago, for TRT that is. Yeah, 2019 was my last show,” said KO.
For O’Hearn, longevity, ligament durability, strength, and movement are the most important aspects of fitness.
“Most of you want to be double nickels like me but still kind of healthy and got good blood markers and all that,” said O’Hearn. “I guess that would be a great thing [my blood work] but also common sense also shows, the strength is still there, the joint and the ligaments are still there and that’s not arguable no matter what you think.
Being able to move, being able to tussle still, being able to do what we love, and I know that you boxed and you’ve done stuff right? And you’re such a puppy right now, but imagine twice your age being able to still do that. That’s what in my belief health and fitness should be about,” said O’Hearn.
While he’s promoting hormone optimization, O’Hearn claims he’s not endorsing TRT use.
“Wait, are you on TRT or just promoting TRT? asks KO.
“I’m promoting optimum health. We answered that. Don’t keep going back. I’m showing you the blood work so you can go with it and then you can make your decision to the fans,” said O’Hearn.
“Yes or no, are you trying to optimize your hormones?”
“Are you trying to get views, he’s trying to get views. I’m not going to give you that! Let’s wait till they see the blood work, let them have something don’t give them too much right now. I’m not going to give that to you kid,” O’Hearn shared.
O’Hearn went on to explain that over decades he never failed a drug test or blood test.
Regardless of how I competed and did my drug tests through my teens and 20s and 30s and 40s and still doing those drug tests and coming up natty, now is that wrong? That a natty guy is promoting a company that does HRT to you and your viewers?” asks O’Hearn.
“I think if you’re transparent and honest about that maybe. It’d be fine if you were maybe a lifetime natural and then now you promote HRT you start your HRT as long as you’re transparent and honest about that, it’d be totally fine,” replied KO.
Overall, O’Hearn wants to see blood work become a regular part of everyone’s routine.
“His testosterone was low, his estrogen was high, the main thing is this is why I teamed up with Titan Medical, because everybody should get blood work. If you get blood work and the blood work looks good, cool, you’re good. And you’re good no matter what age you’re at, but if it’s bad, get help,” explained O’Hearn.
“My guess is that you’re definitely on TRT but that’s my guess. But we will wait until you reveal the answer when the time is right and you can send me the blood work,” said KO.
Lastly, O’Hearn underlined that he hasn’t failed drug tests yet athletes such as Jon Jones and Lance Armstrong have.
“Why don’t you ever explain to the people how I did drug tests, blood tests, from my teens till my 50s and never come up negative.”
“People pass tests all the time, look at Jon Jones, UFC, Lance Armstrong,” said KO.
“Jones got busted. Lance Armstrong got busted,” said O’Hearn.
“Both those guys lasted 10 years I’ve been doing this 40 years. I love it, he just said I’m really really smart and I know how to get past a drug test.”
This wasn’t Mike O’Hearn’s first interaction with Kenny KO face-to-face. The two met last year to discuss O’Hearn’s lifetime natural status. O’Hearn stressed that he looks the way he does at 53 not because of what he does now, but due to the practices he adopted as a teenager. Other online personalities, like Joe Rogan, have also brought attention to O’Hearn’s ‘natty or not’ status, but, like everyone else, they are unsure.
Whether or not O’Hearn is using steroids remains a topic of discussion, with Kenny KO strongly suggesting the use of TRT. The full story may become clearer if O’Hearn decides to release his teased blood work results.
